To become a Sales Partner with Florida Blue with the ability to sell to groups only, you must complete the four-page Request for Appointment form below. Once we determine that you are properly licensed with the FLDFS, and our Sales Director reviews your application, we will contact you with further instructions.
Please fill out the application, print it out and send it to us via email to channelpartnerdatamanager@bcbsfl.com (be sure to include your correct email address on the application). We look forward to working with you.
